Student Travel Awardees

Awardees will need to provide us with the following:

Name

Mailing address

Confirmation of US citizenship

SSN

If an awardee is not a US citizen , they will need to print and complete the UDW8 form (Section 2: Visitor Information) and provide a copy of the appropriate documentation (Visa type) as listed on page 2. In the unlikely event they do not have a SSN, the awardee will need to print and complete Payee Information portion of the P-1 form and also provide a copy of their Visa type.

All the information (with original signatures) should be sent directly to Susan Phipps [phipps@dbi.udel.edu], who will prepare the necessary forms to process payments to these individuals.
